Injection - Shoulder

Try one sub-achromial injection pre-referral.

If you think that alternative routes of shoulder injection are indicated thenplease consider referral.

  1. Mark the injection site by following the scapular spine to the tip of the acromion process and mark the space just below the acromion with your thumb nail. Then sterilise with iodine, your thumb nail mark will still be visible and injection can be achieved using a non touch technique.
  2. Draw up 2mls 1 or 2% lignocaine with 40mg Triamcinolone acetate (Kenalog). Place needle on the thumb mark ands gently insert needle into sub-acromial bursa The injection should be achieved with the minimum of pressure on the syringe. (Simon ideal photograph)
  3. Encourage shoulder movement after injection. Often full movement can be achieved by the time the patient leaves the clinic.
  4. Repeat only once pre-referral unless there is considerable benefit.
