Intravenous Iloprost for Raynaud's

Protocol for Rheumatology Daycase Patients
Dose
Iloprost 50mcg to be given by I.V. infusion on five consecutive
days. (Please note this is an unlicensed product)
On Admission
- Check that the daycase card or patient notes do not indicate
that a medical review is required.
- Obtain baseline observations of pulse, temp and BP.
- Iloprost prescribed by medical staff on i.v. card.
- Analgesia and anti-emetic prescribed by medical staff and administered
prior to infusion commencing to help reduce side-effects of nausea
and headache.
Infusion
- Iloprost 50mcg. Add to 250ml saline 0.9%.
- Day 1:
- Day 2: As day 1.
- Day 3: As day 1
- Day 4: Start at MAX rate tolerated and infuse over 6 hours.
Observations to be checked hourly.
- Day 5: Start at MAX rate tolerated and infuse over 6 hours.
Observations to be checked hourly.
Side Effects/Patient Advice
- Facial flushing.
- Headache
- Nausea and vomiting
- Tachycardia
- Lowering or increase in blood pressure
- Abdominal cramps
Follow Up
- Home after 1 hour following end of infusion if well and BP
and pulse satisfactory.
- Consider keeping patient in for a few extra hours if unwell.
Contact medical staff if in any doubt.
- Arrange OPD review for 4 months if patient does not already
have one arranged.
